Student 575 Course Forms
Once you're enrolled in a 575 internship course, you'll be required to follow all instructions, and complete all forms for this internship course.
START HERE FIRST!
General
Information, and Instructions for the Report.
Read
carefully, then complete the forms below.
Submit to the proper
offices by the given deadline dates.
Failure to submit completed forms at the start and end of the quarter
will delay in you receiving course credit until all paperwork is
received. If not, a NC may
be given by your Faculty Internship Coordinator.
In addition to the Student Agreement Form and Student Reporting Form you should have completed when requesting to be enrolled in the 575 course, complete the following:
BEGINNING FORMS - Forms prior to beginning your internship (be sure you've read the instructions hyperlinked above) COPIES TO: Faculty Intern Coordinator, and to the CBPA Internship Office.
MID-TERM WRITING ASSIGNMENT. There's a one-page writing assignment due during the 5th week of your 10-week internship. Submit your report directly to your Faculty Internship Coordinator by email.
ENDING FORMS - At the end to complete your internship (Reminder: Report due during 9TH WEEK to Faculty Intern Coordinator) COPIES TO: Faculty Intern Coordinator, & to the CBPA Internship Office (just forms)
